Please take note of the following information for your guidance. We are providing this notice to confirm the information you have supplied and recorded on CebuDoc's CFW Portal.

Important Information:

Please present a copy of the prescription issued by your physician. CWC renders services based only on the presented and issued prescription.

It is understood that the chosen date and time for your procedure has been confirmed through our online portal, text, email, or call.

CWC expects payment of the calculated fees through QR ph, online bank deposit or transfer. Proof of receipt which will be confirmed at our end via the online portal, or through text, email, or call.

Please remember that your scheduled appointment is reserved exclusively for you at a specific time, date, and place, and cannot be altered or changed. If you find it necessary to reschedule your appointment, please be advised that any changes made within seven (7) days of the original booking, please remit a rebooking fee of 100 pesos. To ensure that your chosen spot is secured, kindly notify us of any changes as early as possible.

For patients who do not show up for their appointment: Refund requests must be submitted within seven (7) calendar days. If a refund request is not submitted within this timeframe, the refund amount will be forfeited.

Refunds for less than 5,000 pesos will be processed in cash on the same day. For 5,000 pesos or above, refunds will be issued by cheque and processed within five (5) working days.

For patients who are to undergo mammography procedures:
• Do not apply deodorant, talcum powder, lotion, or perfume near the breasts or under the arms.
• Wear comfortable and loose clothing.
• If previous mammograms were done, bring copies and results for comparison.

For obstetric ultrasound: the procedure requires no special preparation since only your lower abdominal area needs to be exposed for the test. It is advisable to wear a loose-fitting two-piece outfit. Please leave any jewelry at home.
